Description of Human Immunoglobulin G4 (IgG4)
Human Immunoglobulin G4 (IgG4) is a 146 kDa protein. Serum IgG4 is the least abundant subclass of IgG antibodies, accounting for only 3–5% of total IgG in circulation. IgG4 possesses unique structural features, notably its ability to undergo Fab-arm exchange, resulting in bispecific antibodies with two different antigen-binding sites. Functionally, IgG4 is distinct from other IgG subclasses because it does not activate the complement system and has limited capacity for antibody-dependent cell-mediated cytotoxicity. Instead, IgG4 primarily acts as a blocking antibody, modulating immune responses and limiting inflammation. This immunomodulatory role is particularly evident in chronic antigen exposure, such as during allergen immunotherapy, where IgG4 levels rise and help suppress IgE-mediated allergic reactions. Elevated IgG4 responses are also characteristic in parasitic infections like schistosomiasis and lymphatic filariasis. Clinically, IgG4 is central to IgG4-related disease (IgG4-RD), a fibroinflammatory condition affecting multiple organs, and is implicated in certain autoimmune diseases where IgG4 autoantibodies disrupt normal tissue function. Additionally, combined IgA-IgG4 deficiency has been linked to recurrent bacterial infections due to impaired immune defense.
