Description of Human Butyrylcholinesterase (BChE)

Human Butyrylcholinesterase (BChE) is a 340 kDa protein. Butyrylcholinesterase (BChE), a tetrameric glycoprotein within the serine hydrolase family, exhibits broad substrate specificity beyond its canonical role in hydrolyzing acetylcholine. Its enzymatic repertoire includes aryl acylamidase activity, protease enhancement, and detoxification of neuromuscular blockers like succinylcholine. Genetic polymorphisms in the BCHE gene produce variants with altered activity, including silent forms linked to prolonged apnea during anesthesia. While once considered functionally redundant, BChE is now recognized for critical roles in neurodevelopment, lipid metabolism, and modulating aggression via ghrelin hydrolysis. Pathologically, BChE accumulates in Alzheimer’s disease plaques and tangles, with altered isoform ratios correlating with disease progression. Clinically, BChE deficiency causes pseudocholinesterase deficiency, impairing metabolism of anesthetics and organophosphorus (OP) agents. Its bioscavenging capacity underpins therapeutic applications: such as the detoxification of cocaine and OP nerve agents. Emerging evidence implicates BChE in metabolic syndrome, with activity inversely correlating with obesity and cardiovascular risk, and SIDS.
